Fluorchinolone: Risiko für Aortenaneurysmen und Aortendissektionen

Oktober 2018 – Die Zulassungsinhaber fluorchinolonhaltiger Arzneimittel informieren in Abstimmung mit der Europäischen Arzneimittel-Agentur (EMA) und dem Bundesinstitut für Arzneimittel und Medizinprodukte (BfArM) über die Aufnahme eines neuen Warnhinweises hinsichtlich des Risikos für Aortenaneurysmen und Aortendissektionen in Zusammenhang mit systemisch und inhalativ angewendeten Fluorchinolonen.

Systemisch und inhalativ angewendete Fluorchinolone können das Risiko für Aortenaneurysmen und -dissektionen erhöhen, insbesondere bei älteren Personen. Bei Patienten mit einem Risiko für Aortenaneurysmen und -dissektionen sollten Fluorchinolone nur nach sorgfältiger Nutzen-Risiko-Abwägung und Berücksichtigung anderer Therapiemöglichkeiten angewendet werden.

Prädisponierende Faktoren für Aortenaneurysmen und -dissektionen sind unter anderem: Aneurysma-Erkrankung in der Familienanamnese, vorbestehendes Aortenaneurysma oder vorbestehende Aortendissektion, Marfan-Syndrom, vaskuläres Ehlers-Danlos-Syndrom, Takayasu-Arteriitis, Riesenzellen-Arteriitis, Morbus Behçet, Hypertonie und Atherosklerose. Patienten sollten über das Risiko für Aortenaneurysmen und -dissektionen informiert werden und dazu aufgefordert werden, bei plötzlich auftretenden schweren Bauch-, Brustkorb- oder Rückenschmerzen unverzüglich in der Notaufnahme ärztliche Hilfe in Anspruch zu nehmen. Patienten sollten sich darüber im Klaren sein, dass es sich bei den angesprochenen Aortenaneurysmen und Aortendissektionen um ganz schwerwiegende, potenziell tödlichen Nebenwirkungen handelt.

Das BfArM hat dazu soeben einen Rote-Hand-Brief zu systemisch und inhalativ angewendeten Fluorchinolonen und dem Risiko für Aortenaneurysmen und Aortendissektionen versendet.
